Dear Parents,
Due to state regulations, we need to obtain parental permission for each child to have their picture taken, as well as to take walks around the center. Please fill-out the permission slip below making sure to circle your choice and return them to the tuition mailbox located in the reception area. Siblings must have separate permission slips.
